Outreach and Enrollment Team at OHI Is Scheduling Appointments for Open Enrollment

The Outreach and Enrollment team at (Ocean Health Initiatives), a federally qualified health center, are scheduling appointments to help the general public and its patients with enrollment in the Health Insurance Marketplace, which begins November 1, 2015, and ends January 31, 2016.

Sasha Andino and Carmen E. Lopez, associates of the Outreach and Enrollment Department, and other qualified OHI staff will answer questions, explain any changes, and help individuals sign up for health insurance. Bilingual certified application counselors are available by appointment only.

Appointments can be made at four OHI sites: Lakewood Health Center, 101 Second Street, Lakewood; Toms River Health Center, 301 Lakehurst Road, Toms River; Manahawkin Health Center, 333 Haywood Road, Manahawkin; Manchester Health Center, Lakehurst Circle Center II, 686 Route 70, Lakehurst.

To sign up for health insurance at the time of their appointment, enrollees should bring their social security card; proof of income, such as paystubs, W-2 forms, or other income information; their employer’s information; and their date of birth. Eligible immigrants who want health coverage should bring their documents.

Visit OHI at the 2015 Elderfest Expo

Free Screenings and Outreach and Enrollment Information

Ocean Health Initiatives (OHI), a federally qualified health center, is participating in the Elderfest Expo hosted by Preferred Home Health Care & Nursing Services, Thursday, November 5, 2015, at the Clarion Hotel and Conference Center, 815 Route 37, Toms River, N.J., from 11:00 a.m. to 3:00 p.m.

The Mobile Medical Unit of OHI, staff, and members from the Outreach and Enrollment Department will provide free glucose testing and education about its medical services in Ocean County. Certified navigators from the Health Insurance Marketplace of OHI will be available to answer questions about upcoming open enrollment, and to assist the public with outreach and enrollment information.


Health Insurance Eligibility is Available for Select Situations

OHI would like to remind you that some individuals and families may still be eligible to enroll in health insurance.  Even though the formal open enrollment period has closed, our Outreach and Enrollment Department is available to assist our patients and the public with any questions about the Health Exchange.

Special Open Enrollment in the Marketplace is available up to 60 days AFTER the following events have occurred:Bigger-Family

  • Those who are recently married
  • Had or adopted a child
  • Had a change in immigration status
  • Have been released from incarceration
  • Received an exemption from the Marketplace but now have a higher income
  • Anyone who lost health insurance because of:

– Leaving or losing a job
– Moving into New Jersey or moving from another part of New Jersey
– Divorce
– Turning 26 years of age and lost coverage on a parents plan
– Lost eligibility for Medicaid or CHIP

These life-changing situations allow people to participate in this special enrollment period; however, it is important to remember that you must enroll within 60 days after the event occurred.

People who can enroll ALL year include:

  • Anyone eligible for Medicaid or have a child who is eligible for CHIP (Children’s Health Insurance Plan)
  • A small business owner who wants to offer coverage to employees
  • Native American or Alaskan native

OHI Offers Assistance as Health Insurance Deadline Nears

3-29-14 - Open Enrollment Closing EventIf you are waiting for the last minute – it’s almost here. Monday, March 31, 2014, is the last day to enroll for health insurance. On Saturday, March 29, 2014, certified navigators in the Health Insurance Marketplace of OHI, a federally qualified health care center, will be available to assist the public by appointment only from 8:00 a.m. to 4:30 p.m., 101 Second Street, Lakewood, N.J.

Besides registering for health insurance, OHI is offering a number of health-related activities throughout the day, such as free health screenings, including blood pressure checks, glucose screenings, and diabetic education. Representatives from SNAP (Supplemental Nutrition Assistance Program), a government assistance program to help low-income households pay for food, will be available to offer assistance.

OHI is also sponsoring a food drive to collect canned goods and nonperishable items for a local food bank or Catholic Charities. Children can enjoy face-painting, taking pictures with a cartoon character, and animal balloons. Free light refreshments (granola bars, fruit, donuts, water and soda) will be available.

3-29-14 - Open Enrollment Closing Event (Spanish)Beatriz C. Patino, the coordinator of the Health Exchange Marketplace Department, is hoping to enroll at least 500 people by the end of the month. OHI Extends Its Hours for Open Enrollment Appointments

With the deadline to enroll in health insurance looming, OHI (Ocean Health Initiatives), is extending the hours of its Open Enrollment Help Days in Lakewood, Toms River, and Manahawkin to accommodate the growing need of the general public and OHI patients to enroll in health insurance through the Affordable Care Act.2-21-14 - Extended Hours

Bilingual Certified Application Counselors in the OHI Health Exchange Marketplace department will be available by appointment only through March 31, 2014 as follows:

1. Toms River, 301 Lakehurst Road:
Monday – Thursday: 8:00 a.m. to 7:00 p.m.
Friday: 8:00 a.m. to 3:30 p.m.

2. Lakewood, 101 Second Street:
Monday: 8:00 a.m. to 6:00 p.m.
Tuesday – Thursday: 8:00 a.m. to 7:00 p.m.
Friday: 8:00 a.m. to 7:00 p.m.
Saturday: 8:00 a.m. to 3:30 p.m.

3. Manahawkin at 333 Haywood Road:
Monday, Wednesday, Thursday, Friday: 9:00 a.m. to 4:00 p.m.
Tuesday: 9:00 a.m. to 7:00 p.m.

Health Exchange Marketplace Open for Ocean County

OHI (Ocean Health Initiatives), a federally qualified health care center, with locations in Lakewood, Toms River, and Manahawkin, has established a Health Exchange Marketplace Department to assist the public in understanding information about affordable health insurance coverage in advance of the Affordable Care Act, which begins on January 1, 2014.



The department is now made up of Beatriz C. Patino, Brick, the coordinator of the Health Exchange Marketplace, and Carmen E. Lopez, Neptune City, an associate, who will also answer questions and explain any changes. Additionally, they will help individuals sign up for health insurances during the open enrollment period, October through March, and determine what plans would be best for individual situations.

Both Patino and Lopez, who are bilingual, can be reached in the Health Exchange Marketplace Department, Lakewood, at 732-363-6655. They will also assist the public at Toms River and Manahawkin sites, and will be available at many outreach events.

Patino has served as a community health worker in the Central Jersey Health Consortium, Lakewood and North Brunswick. There she performed community outreach for such groups as the VNA, WIC, Women’s Wellness Centers, hospital clinics, OBGYN offices, and several resource centers. Patino earned a B.A. in political science from Kean University in 2005 and anticipates earning her Masters of Social Work Clinical Practice with Families and Children in 2016 from Kean University.

Lopez, a certified medical assistant (CMA), has had wide experience in a multi-physician medical practice and in hospital settings. Before joining the Health Exchange Marketplace, Lopez worked as a CMA at Loyal Source Government Services, Fort McGuire Air Force Base, and at the N.J. Urological Institute, formerly Shore Urology, Eatontown.
