Open enrollment dates for the 2018 period have been released for individuals and families to apply for health care through the Affordable Care Act (ACA). Ocean Health Initiatives, Inc., (OHI), a federally qualified health center with locations throughout Ocean and Monmouth County, is scheduling appointments with its Outreach and Enrollment Team to assist in the application process. The enrollment period begins on November 1st, 2018 and extends through December 15th, 2018.
Bilingual certified application counselors are available by appointment only to answer questions, explain changes and help individuals sign up for health insurance. Appointments can be scheduled for any OHI Health Center.
To sign up for health insurance at the time of an appointment, enrollees must bring their social security card; proof of income, such as paystubs, W-2 Forms, or other income information; their employer’ information; and their date of birth.
