Enrique Teutelo - Beatriz Patino - Theresa BergerDr. Theresa Berger, CEO of Ocean Health Initiatives (OHI), a federally qualified health care center (FQHC) with locations in Lakewood, Toms River, and Manahawkin, and Beatriz Patino, OHI’s Health Exchange Coordinator, can be seen December 1, 2013, on the Spanish-speaking television station Univision Communications Inc., Channel 17*, at 7:30 a.m.

Enrique Teutelo, a newscaster at Univision, interviews Dr. Berger and Patino about OHI, the services offered to underinsured and uninsured individuals; how OHI can help people sign up for health insurance during the open enrollment period, October through March; and how to determine which plans would be best for their individual situations.

OHI has established a Health Exchange Marketplace Department to assist the public in understanding information about affordable health insurance coverage in advance of the Affordable Care Act, which begins on January 1, 2014.

Beatriz Patino - Theresa Berger (TV)Dr. Berger, who is bilingual in Spanish and English, has been the Chief Executive Officer (CEO) of OHI since April 2003. Under her leadership, OHI has opened additional sites in Toms River and Manahawkin, has added a dental department, and 2 school based programs.

Patino, has served as a community health worker in the Central Jersey Health Consortium, Lakewood and North Brunswick. There she performed community outreach for such groups as the VNA, WIC, Women’s Wellness Centers, hospital clinics, OBGYN offices, and several resource centers. Patino earned a B.A. in political science from Kean University in 2005 and anticipates earning her Masters of Social Work Clinical Practice with Families and Children in 2016 from Kean University.

shutterstock_89752360Ocean Health Initiatives has developed a multi-discipline strategies approach for addressing the needs of the chronically ill patient.

We have developed a true team approach which incorporates Provider, Pharmacist, nursing staff, Behavioral Health and the Chronic Care Coordinator. We meet and review charts for patients that have visited our facilities for a specific disease state. We currently have three Populations of Focus (POF) that we closely monitor: Diabetes, Asthma and Hypertension. Based on the findings of our weekly meetings we bring patients in for either a nurse’s visit or an appointment to see their provider.

The patients meet individually with the Pharmacist and Chronic Care coordinator; they are counseled on proper medication usage, dietary recommendations, and physical activities. At this time we refer them to the Chronic Disease Self-Management Program (CDSMP) if appropriate. We offer “Take Control of Your Health” in both English and Spanish, as well as “Take Control of Your Diabetes” in English and will be sending staff to cross-train in the Spanish version of the diabetes CDSMP.

We are building a new approach for addressing the needs of our high risk patients, attempting to be proactive to their concerns and minimizing multiple provider appointments for recurring issues.

Lessons learned:
• At times we must do a soft handoff to a service such as the laboratory in order to achieve a higher compliancy rate for definitive test results

• All staff is involved in patient education at all levels or areas of the patient visit resulting in improved outcomes

• We have moved to an open access appointment method resulting in higher patient satisfaction

• We have incorporated the Patient Centered Care Navigator whom calls each patient after visit to see if all needs were met and to see if any issues remain unresolved helping improve compliancy rates for provider’s plan of care

• Patients who receive one on one interaction from empathetic staff members show improved outcomes

Since costs is a primary concern among our under and non-insured patients we have instituted a 340B program and partnered with a local pharmacy to deliver prescribed medications at a substantial reduced rate. We have also arranged with our partnered laboratory service to perform certain required analysis at a reduced or discounted rate.

Dr. Theresa Berger, CEO of OHI, said, “Health care drives education. The key is to keep the children healthy so they do not miss school. The less sick days children take, the more time they will be in the classroom. This translates into a better education, which sets the groundwork for a better future for our children.”

OHI Changing Diabetes Testing Day at Toms River Site

OHI (Ocean Health Initiatives), is changing its diabetes A1c testing day at its Toms River site, 301 Lakehurst Road, Toms River, to Thursday, November 14, from noon to 7:00 p.m. instead of Friday, November 15.

Diabetes testing will be still be given noon to 4:30 p.m., Wednesday, November 13 at 333 Haywood Road, Manahawkin.

Each site will have diabetic supplies and giveaways; and Israel Arroyo, chronic care coordinator for OHI, will present a half-hour educational seminar Managing Your Diabetes at 3:00 p.m. on all testing days. OHI pharmacists Vijay Vohra and Nancy Karroum will be available to discuss diabetic medication management. Most insurances are accepted.

OHI to offer flu shots at Health Fair, Georgian Court University

OHI (Ocean Health Initiatives), a federally qualified health care center, with locations in Lakewood, Toms River, and Manahawkin is offering flu shots during a Healthy Campus Health Fair, Tuesday, November 12 from 10:00 a.m. to 3:00 p.m., at the Casino, Georgian Court University, 900 Lakewood Avenue, Lakewood. No appointments are required.

Most insurances are accepted. For those without insurance, flu shots cost $20.00 for the regular vaccine. The cost of flu shots are payable with cash or checks. Anyone receiving a flu shot must have a photo I.D.

OHI will provide information about its services and staff from the Health Exchange Department will help students understand how to navigate the New Jersey Health Exchange Marketplace.

OHI Offers Flu Shot Clinic at Lifestyle Expo

shutterstock_116440492OHI (Ocean Health Initiatives, Inc.) is offering a Flu Shot Clinic at the Bob and Marianne’s Lifestyle Expo, Friday, November 1. 2013, from 10:00 a.m. to 2:00 p.m., Center Court, Ocean County Mall, 1201 Hooper Avenue, Toms River.

The vaccine is free for attendees with a valid Medicare Part B card and a photo I.D., while supplies last. Most insurances will be accepted. For those without insurance, flu shots cost $20.00 for the regular vaccine and $30.00 for the high dose flu vaccine available only to those 65 years of age and older. The cost of flu shots are payable with cash or checks. Anyone receiving a flu shot must have a photo I.D.

The Lifestyle Expo is presented by 1160 WOBM-AM and Meridian Health. Admission and parking are free to the Expo, which features free information and exhibits.
